I dunno, looks real to me. *shrug* Of course he looks old/like Chiyo, when you consider his actual age. Did we expect Madara to actually look like a young child? If this proves to be real, it means Madara never body hopped, pretty much killing the Obito theories, and shows that Madara did indeed find a way to become immortal at some point in his natural old age. Itachi did state that Madara is immortal, after all.
