I think the blade was made of metal that Pains body can make at will and use as weapons or piercings, also used as chakra recievers. Metal chakra made chakra receivers. I think he stabbed Hinata, just left her alive enough to suffer or to suffer through losing her love as he kills Naruto by extracting Kyuubi. It may be blood mixed with chakra, similar to the Irond Sand kage or the guy with the Bones kekkegenkai, but i definitely think it's a manipulation of some type of chakra metal.
