Bwahahaha...mayroon din akong matutulungan dito, salamat ulit sa sig ko.

Anyway, halos pareho rin ang O.R. dito at diyan.Nagtrabaho rin ako sa O.R. diyan sa P.G.H. kaya alam ko rin ang difference.Anyway, I can do both-scrub or circulate at may dagdag pa...lol, nagaassist ako ng doctor dito. Marunong akong mag suture ng balat, mag tie ng suture at gumamit ng lahat ng instrumento sa O.R. habang nag a assist sa duktor.
First of all, alam mo dapat kung ano ang sterile at unsterile at papaano mo maiiwasan ma contaminate ang sterile field.I'm sure may libro diyan para maexplika ito.Magiging nobela itong post ko kung sasabihin ko lahat..lol.
Next, dapat marunong kang mag scrub ng kamay mo.Ideally, first scrub is 7-10 min.tapos succeeding is 5-7. In the real world, hindi nasusunod ito parate lalo na kung nagmamadale ka...lol.At saka, mas maraming chemicals na available besides betadine na mabilis ang epekto pumatay ng bacteria.
Next dapat marunong kang mag suot ng gown and gloves .
Vocabulary:

mosquito- clamp para tumigil yung bleeder
Metz- gunting
tie- pang tie sa bleeder. usually dito silk ties at chromic ties, pero diyan baka cotton ties pa...lol

rule of thumb-- clamp, clamp , cut, tie-- ganoon ang sequence.Hindi na dapat sasabihin sa yo, automatic na yon na kapag humingi ng clamp [mosquito,or kelly (isa pang klase ng clamp-mas malaki ng kaunti sa mosquito) ] clamp ulit ang kasunod, tapos Metz para gupitin yung tissue, tapos tie para i-tie yung bleeder, followed by another scissors called mayo para gupitin yung extra tie.Habang nagtatie yung assistant. ibibigay pabalik sa yo yung clamp tapos bigay mo naman yung mayo para gupitin yung extra tie.Tapos uulitin ulit para doon naman sa kabilang side na may clamp .

marami nga akong matutunan. dito ang tie is suture. hehehe...ganda naman ng work mo. nyaaaa...wonder if i'll be as pro as you are. ^^ same nga ang nangyayari dito, kahit medical handwashing or bed making hindi na na-fo-follow. and we had to go through all those nerve wrecking return demonstrations sa mg C.I. namin. hehehe...salamat sa input. i'll ask more once i'll be exposed in the O.R. hanggang surgical lecture pa kasi kami. somewhere around april kami mag-start ng O.R. exposure. by then, i shall try to take down questions for you. nyahhaaha...

and i hope i can keep the sterility of the instruments. sigh...dapat talaga isasaulo at isasadibdib ang principles of sterility. -_-;; btw, i dunno kung mosquito ang tawag sa clamp sa amin, plain clamp lang kasi term namin. hehehe...

oh yeah..what about the counting of instruments? how does that follow there? sa lecture kasi namin we were given this pneumonics to remember in the initial and final counting of instruments. e.g. in initial, 1st, 2nd & 3rd counting, we have to remember INS (instruments, needles & sutures) while in the final counting (tapos ng procedure), we follow the SIN in reporting the counting to the surgeon. same ba jan? ^^

Sa counting ...may count sheet dito.Back to back page. Front page- listahan ng mga small items to count ...

Laps,mini laps, raytecs (x-ray detectable sponges), cottonoids, peanuts, blades,needles,bovie tips, hypodermics,clamp covers, irrigating tips,..etc.
sample lang ito- naka grid ang list na ito into first 2nd and 3rd count .Tapos may extra space kung may dagdag na items para hindi magulo ang count.

Tapos sa next page- intrument counts naman.Classified sa scissors, clamps, bobcocks, kochers, etc....nakagrid din.
Tapos kung ok lahat ng count, inaannounce sa surgeon.So ganun din.Pero alam mo nung nasa PGH ako.scrub lang ang nagcocount kasi toxic ang or palagi.Walang circulating all the time.Circulating ay circulating sa 6 or 8 rooms. Grabe!!Pero dito talagang 2 people per case, sometimes 3 pa...lol.Oks talaga.Maging O.R. ka na lang, kailangan dito niyan dahil walang masyadong may gusto dahil kadiri raw...lol.Pero masarap kasi maraming O.T. at hindi kasing toxic ng floor nursing, at the best thing...hindi ka nagpupunas ng puwet every single time...lol!
