Kakashi's really isn't unique. Because the head ninja for the Cloud Village (who is now deceased) wore his forehead protector the same way, but the different eye.

That would be Huga_Tenten, after nearly three mounths of death...anyway, I think the most unique is a tie between Gaara's and Sakura's. Sakura is the only one who has it on top of her head.
