Four days into his hospitalization, he had a grade I aortic dissection, but Teddy decided to monitored it in the hopes it would resolve on its own. It didn't and later transected. Teddy collapsed before she could start operating on him and Simone and Lucas decided to do the planned thoracotomy. They were unable to stop the bleeding and Sam died in the OR.
